A GUIDE TO THE MEDICAID APPLICATION
Our office strives to provide the best possible service in assisting you to understand the Medicaid application process. In order for your application to go smoothly, we offer the following suggestions:
IT IS CRITICAL THAT YOU ARE FULLY PREPARED WHEN YOU GO TO THE MEDICAID OFFICE TO MAKE APPLICATION:
- Use the individualized Asset Protection Plan we prepared for you as a guide as to when you can apply for benefits.
- Complete the application as accurately as possible. Remember, you can get into trouble and be denied benefits when you try to hide income, assets, and transfers, not when you provide the information and can show how you have followed the rules
- If transfers were made during the previous 36 months (60 months to or from trusts or since February 2006), list them accurately including date and amount. Be prepared to show how and why transfers were made.
- Have all your documentation ready - include not only the originals but a full set of copies for the caseworker. You will need the following information:
- Social Security card
- Medicare card
- Health insurance cards
- Birth certificate (or naturalization papers)
- Marriage certificate
- Death certificate (if spouse is deceased)
- Life insurance policies (if any)
- Statements of all resources for 36 months (60 months for trusts)
- Bank accounts, CD's, IRA's, stocks, bonds
- Deeds to property other than primary residence
- Deed to primary residence
- Proof of income (pensions, SS, interest)
- Health insurance premiums
- If married, copies of household expenses (rent/mortgage, utilities, insurance, taxes)
- Funeral arrangements, pre-paid funeral plan, cemetery deed
- Car registration
- If married, have the value of all resources as of the date of institutionalization of the spouse - this will allow the Medicaid Office to determine the Community Spouse Resource Allowance.
Any of the above items you may not have with you at the initial meeting must be provided within 30 days. The Medicaid Office will assign a caseworker to your file who will be your contact through the application process.
